The City of Canandaigua has death records of anyone who died within the City of Canandaigua. Certified copies may be obtained by parent, spouse, child or sibling by following the procedure listed as follows:

Funeral Homes: If applying for the certificate through the online portal, please include your identification and where you would like the documents sent or picking up the copies in the office. The cost is $10 per requested copy. Please apply here: Death Record Application

You can apply in person at the City Clerk's Office during office hours, 9 am to 4 pm, Monday through Friday. The fee for a certified copy is $10 per copy requested.
Family relationship may need to be verified - for example, a daughter requesting a copy of her father's death certificate would also need to provide a copy of her birth certificate showing her father's name.
Death records are also available to attorneys or for any lawful legal purpose which must be verified through documentation. Please reach out to the City Clerk for more information.
